Physician Services

Advance Hospice Care’s specially-trained, board-certified physicians are available to visit patients where they live – whether at home, in a nursing home, or in an assisted living facility – for a primary consultation and follow-up visits. Our doctors complement the services of the attending physician to meet the general medical needs of patients and the emotional needs of their families, and also collaborate with the primary health care provider to answer questions, manage pain and other symptoms, and help the patient live as well as possible through serious illness.

Physician Service Care Includes:

  • Consultation services with a physician and a written report of recommendations for symptom management options
  • Helping patients and their physician manage pain and other symptoms, such as fatigue, shortness of breath, nausea, constipation, loss of appetite, or difficulty sleeping
  • Reviewing medical information and working with the patient and their family to assess treatment options so that they can make decisions that fit with their priorities and goals
  • Supporting patients and families through all of the challenges of coping with serious illness
  • Assisting in planning for future care, such as helping determine the most appropriate care setting
